What ICD-10-CM says

Notes at F43.2, Adjustment disorders (they apply to every code under it)

Includes

  • Culture shock
  • Grief reaction
  • Hospitalism in children

Excludes2 (not part of F43.22; both may be coded)

  • separation anxiety disorder of childhood (F93.0)

What F43.22 covers

F43.22 is for an adjustment disorder in which anxiety is the main symptom in reaction to an identifiable stressor. Whether the DSM-5-TR criteria are met is the clinician's call.

Behavioral health and primary care use it. F43.23 is for anxiety with depressed mood, F43.21 for depressed mood alone and F43.20 for an unspecified adjustment disorder. Generalized anxiety disorder (F41.1) is not tied to a specific stressor.

Billing and coding notes

The F43.2 subcategory includes grief reaction and culture shock, and has an excludes2 for separation anxiety disorder of childhood (F93.0). Adjustment reaction is also an excludes2 at F41, so a separate anxiety disorder can be coded with F43.22 when the clinician documents both.

If the documented symptoms include depressed mood as well as anxiety, F43.23 is the accurate code. If no subtype is documented, use F43.20.
