Free superbill template and generator
A superbill is an itemized receipt your client sends to their insurance company to be reimbursed for out-of-network care. Fill in the form below and download a one-page superbill PDF to give your client.
It's free and needs no account. The PDF is made in your browser: nothing you type about your client is sent to CloudMedico.
Fill in your superbill
What you type here stays in this browser. The PDF is made on your device, and the ICD-10 search runs on a copy of the code list downloaded to your browser.
Check before you send it
- Add your NPI and tax ID: insurers need both.
- Add your client's name.
- Add at least one diagnosis: insurers need an ICD-10-CM code.
- Add at least one service with a code and a charge.

Superbills in CloudMedico
If your practice is in the United States, CloudMedico makes the superbill from the invoice. Your NPI and EIN, the client's insurance details, the visit's ICD-10-CM diagnoses and your codes, modifiers and fees are already on it, so you don't type them again.
Your clients can download it themselves from the patient portal once the invoice is paid.

There is no official superbill form. Insurers ask for the details a claim needs, and most expect these:
- Your name and credentials, license number, NPI and tax ID, with the practice's address and phone.
- Your client's name, date of birth and address, and their insurance member ID if they have one.
- The date and place of service of each session.
- The ICD-10-CM diagnosis codes, as specific as the record allows.
- The CPT or HCPCS code of each service, with any modifiers the insurer asks for, and the number of units.
- Your fee for each service, what your client paid and how.
- Your signature and the date.
Place of service codes
The place of service code says where the session happened. These come from the CMS Place of Service code set:
- 11: Office.
- 02: Telehealth provided other than in patient's home.
- 10: Telehealth provided in patient's home.
- 12: Home.
How your client uses it
Your client sends the superbill to their insurance company with proof of payment and the insurer's own claim form, if it has one. Many insurers take claims through their website or app.
The insurer decides what it reimburses, based on the client's out-of-network benefits. A superbill doesn't guarantee payment.
Why this page lists no CPT codes
CPT codes and their descriptions are copyrighted by the American Medical Association, so this page doesn't list them. Type the codes you bill, and describe each service in your own words.
Questions
Is this superbill generator free?
Yes. There is no account and no limit: download as many superbills as you need.
Do you see my clients' information?
No. The superbill is filled in and made into a PDF in your browser. Nothing you type about a client, their diagnoses or their sessions is sent to CloudMedico. If you ask for a blank superbill by email, we receive only your email address and your practice details.
Can one superbill cover several sessions?
Yes. Add a line for each session, with its own date and place of service. Many therapists give clients one superbill a month.
Which ICD-10 codes can I use?
The search uses the CDC's ICD-10-CM list for fiscal year 2027, the one in force since October 1. Insurers want billable codes, the most specific that fits. You can also type any code yourself.
Does a superbill guarantee reimbursement?
No. Your client's insurer decides whether and how much it reimburses, based on their plan's out-of-network benefits.
Is a superbill the same as a CMS-1500 claim form?
No. Providers file the CMS-1500 claim form with insurers. A superbill is a detailed receipt your client sends with their own claim.
