What Z00.00 covers

Z00.00 is for a routine adult check-up or annual physical where no abnormal findings come up. The Z00.0 subcategory covers periodic adult exams and the lab and imaging work done with them.

Primary care and internal medicine use it most. If the exam finds something abnormal, the code is Z00.01, with codes for the findings added. Children's well visits use Z00.12- instead, which is an excludes1 here.

Billing and coding notes

The guidelines say Z00.00 or Z00.01 is chosen from what is known when the encounter is coded. If no abnormal findings were found but test results are still pending, coding "without abnormal findings" is acceptable. Z00 codes are first-listed codes.

The Z00.0 subcategory has an excludes1 for an exam for a sign or symptom: that visit is coded to the sign or symptom, not Z00.00. Chronic conditions and history codes may be added when the exam is routine and not focused on a particular condition.

Questions about Z00.00

What is the difference between Z00.00 and Z00.01?

Z00.00 is an adult exam with no abnormal findings. Z00.01 is the same exam with abnormal findings, and the findings are coded as well.

Can chronic conditions be coded with Z00.00?

Yes. The guidelines allow existing chronic conditions and history codes as additional codes on a routine exam that is not focused on any one condition.
