What R51.9 covers
R51.9 is a symptom code for a headache with no more specific diagnosis. It also includes facial pain NOS. Before FY2021 it was the single code R51, which is now a category.
Primary care, urgent care and neurology use it. R51.0 is headache with an orthostatic component. Diagnosed headache disorders have their own codes: migraine in G43.- and other headache syndromes, such as tension-type headache, in G44.-.
Billing and coding notes
The R51 category has an excludes2 for atypical face pain (G50.1), migraine and other headache syndromes (G43-G44) and trigeminal neuralgia (G50.0). Once a headache disorder is diagnosed, code that disorder. The headache itself is part of it and is not added as R51.9.
R51 can no longer be reported on its own. Claims need R51.0 or R51.9.

Questions about R51.9
Is R51 still a valid code?
No. R51 is now a category. Use R51.9 for headache, unspecified, or R51.0 for headache with an orthostatic component.
Should R51.9 be coded with a migraine code?
Not for the migraine headache itself, since the headache is part of the migraine. The excludes2 at R51 allows both only when there is a separate, unrelated headache documented.

In CloudMedico, a visit's diagnoses come from the current ICD-10-CM list. For US practices, the visit flags a code that isn't billable on its date, and a superbill made from an invoice carries the visit's diagnoses.
