What M54.50 covers
M54.50 is for low back pain when the record gives no more detail about its type. It includes loin pain and lumbago NOS. It replaced the old single code M54.5, which became a subcategory in FY2022 and can no longer be reported on its own.
Primary care, physical therapy, chiropractic and pain management use it constantly. Its siblings are M54.51 for vertebrogenic low back pain and M54.59 for other low back pain. Low back pain that travels down the leg with sciatica belongs in M54.4- (lumbago with sciatica), not here.
Billing and coding notes
The M54.5 subcategory has an excludes1 list, so none of these are reported with M54.50: low back strain (S39.012-), lumbago due to disc displacement (M51.2-), lumbago with sciatica (M54.4-), and lumbar or lumbosacral disc degeneration with discogenic back pain only (M51.360, M51.370). If the provider documents one of those, use that code instead.
As a symptom code, M54.50 is not added when a confirmed diagnosis already explains the pain. If the pain is documented as chronic, G89.29 may be added; the guidelines say to list G89.29 first only when the visit is for pain management.

Questions about M54.50
Is M54.5 still a valid code?
No. Since October 2021, M54.5 is a subcategory and claims need a fifth character: M54.50, M54.51 or M54.59.
Can M54.50 and G89.29 be coded together?
Yes, when the provider documents chronic low back pain. The guidelines allow G89 codes with site codes to add detail. G89.29 goes first only when the encounter is for pain control or management.

In CloudMedico, a visit's diagnoses come from the current ICD-10-CM list. For US practices, the visit flags a code that isn't billable on its date, and a superbill made from an invoice carries the visit's diagnoses.
