What I25.10 covers
I25.10 is for atherosclerotic heart disease of the native coronary arteries when the patient has no angina. The I25.1 subcategory includes coronary artery disease, coronary atherosclerosis and atherosclerotic cardiovascular disease, and I25.10 includes atherosclerotic heart disease NOS.
Cardiology, primary care and internal medicine use it. When angina is present, the combination codes in I25.11- apply instead. Atherosclerosis of a bypass graft or a transplanted heart is coded in I25.7-.
Billing and coding notes
The guidelines let you assume a link between coronary atherosclerosis and angina unless the provider says otherwise. That is why angina moves the patient to I25.11- and no separate angina code is needed. I25.10 is right only when angina is absent or not documented.
The I25.1 subcategory asks for an additional code, if applicable, for coronary atherosclerosis due to lipid-rich plaque (I25.83) or calcified lesion (I25.84). Graft atherosclerosis (I25.7-) is an excludes2, so it can be coded alongside when native vessel disease is also documented.

Questions about I25.10
What is the difference between I25.10 and I25.11?
I25.10 is native coronary artery disease without angina. I25.11- is the same disease with angina, and needs a sixth character for the type of angina.
Is I25.10 the code for CAD?
For coronary artery disease of the native arteries without angina, yes. Check for angina, bypass grafts or a recent MI, which each change the code or add one.

In CloudMedico, a visit's diagnoses come from the current ICD-10-CM list. For US practices, the visit flags a code that isn't billable on its date, and a superbill made from an invoice carries the visit's diagnoses.
