What G89.4 covers

G89.4 is for chronic pain syndrome: chronic pain with significant psychosocial dysfunction. It is a distinct diagnosis, not another way of saying chronic pain.

Pain management, behavioral health and primary care use it. Chronic pain without the syndrome is G89.29 (or G89.21 and G89.28 for trauma and postprocedural pain). Central pain syndrome is G89.0 and neoplasm-related pain G89.3.

Billing and coding notes

The guidelines say G89.4 is different from "chronic pain" and should only be coded when the provider specifically documents chronic pain syndrome. G89.29 cannot be reported with it, because chronic pain syndrome is an excludes1 at G89.2.

The G89 category says to code also related psychological factors associated with pain (F45.42). Pain disorders exclusively related to psychological factors (F45.41) are an excludes1. Site codes can be added for detail, with sequencing based on whether the visit is for pain management.

Questions about G89.4

Can G89.4 and G89.29 be coded together?

No. Chronic pain syndrome (G89.4) is an excludes1 at G89.2, so use one or the other based on what the provider documents.

When should G89.4 be used?

Only when the provider specifically documents chronic pain syndrome. The guidelines say it is not the same as chronic pain.
