What G89.29 covers
G89.29 is for chronic pain that the provider documents as chronic and that is not due to trauma, surgery or a neoplasm. ICD-10-CM sets no time frame for chronic pain, so the provider's documentation decides.
Pain management, primary care and spine practices use it, almost always with a code for the site of the pain. Its siblings cover chronic pain due to trauma (G89.21), post-thoracotomy pain (G89.22) and other postprocedural pain (G89.28). Chronic pain syndrome is a separate diagnosis, G89.4.
Billing and coding notes
The guidelines say G89 codes are not used when the underlying diagnosis is known, unless the encounter is for pain control rather than treatment of that condition. When the visit is for pain management, G89.29 goes first and the site code follows. For other visits without a confirmed diagnosis, the site code goes first and G89.29 second.
The G89.2 subcategory has an excludes1 list that includes chronic pain syndrome (G89.4), central pain syndrome (G89.0) and neoplasm-related chronic pain (G89.3). Only one of these describes the patient's chronic pain, so they are not reported together.

Questions about G89.29
What is the difference between G89.29 and G89.4?
G89.29 is chronic pain. G89.4 is chronic pain syndrome, which the guidelines say should only be coded when the provider specifically documents that syndrome. They are excludes1, so not coded together.
Does G89.29 go first or second?
First when the visit is for pain control or pain management. Otherwise the site of the pain is coded first and G89.29 follows.

In CloudMedico, a visit's diagnoses come from the current ICD-10-CM list. For US practices, the visit flags a code that isn't billable on its date, and a superbill made from an invoice carries the visit's diagnoses.
