What E66.9 covers
E66.9 is for obesity when the record does not give the type, cause or class. It includes obesity NOS.
Primary care, endocrinology, bariatrics and nutrition services use it. More specific options include E66.0- for obesity due to excess calories (E66.01 is morbid obesity), E66.1 for drug-induced obesity, E66.2 for morbid obesity with alveolar hypoventilation, E66.8- for other obesity including the class 1, 2 and 3 codes, and E66.3 for overweight.
Billing and coding notes
The E66 category says to use an additional code for the body mass index, if known: Z68.1-Z68.45 for adults and Z68.5- for children. The guidelines allow the BMI to come from other clinicians' notes, but the obesity diagnosis itself must come from the provider. A BMI code is only reported with an associated diagnosis such as obesity.
During pregnancy, childbirth or the puerperium, the E66 code first note puts O99.21- before E66.9. If the provider documents the obesity class or morbid obesity, use that code instead of E66.9.

Do I need a BMI code with E66.9?
When the BMI is known, yes. The E66 category says to use an additional Z68 code for the BMI.
What is the difference between E66.9 and E66.01?
E66.9 is obesity with no further detail. E66.01 is morbid (severe) obesity due to excess calories, used when the provider documents morbid or severe obesity.
