What E11.9 covers
E11.9 is for type 2 diabetes when the record documents no complications. The E11 category also includes diabetes NOS, so if the type is not documented, the guidelines default to E11.
It is one of the most common codes in primary care, endocrinology and internal medicine. When a complication is present, a more specific E11 code takes its place, such as E11.65 for hyperglycemia, E11.22 for diabetic CKD or E11.40 for diabetic neuropathy. Type 2 diabetes in remission without complications is now E11.A, which is an excludes1 here.
Billing and coding notes
The E11 category asks for an additional code for how the diabetes is treated: Z79.4 for insulin, Z79.84 for oral antidiabetic or hypoglycemic drugs, and Z79.85 for injectable non-insulin drugs. A patient with type 2 diabetes who uses insulin is still coded E11, not type 1.
The guidelines presume a link between diabetes and the conditions listed under "with" in the Alphabetic Index, such as CKD and neuropathy. So a patient with type 2 diabetes and CKD gets E11.22, not E11.9 plus an N18 code alone, unless the provider documents another cause.

Questions about E11.9
Can E11.9 be coded with Z79.4?
Yes. The E11 category asks for a Z79 code to show treatment, so Z79.4 goes with E11.9 for a type 2 patient on long-term insulin. It is not used for insulin given briefly to bring blood sugar under control.
What is the difference between E11.9 and E11.65?
E11.9 is type 2 diabetes with no documented complications. E11.65 is type 2 diabetes with hyperglycemia, which the Index also uses for diabetes described as poorly controlled, inadequately controlled or out of control.
